Once you have been accepted for a position with Avsource Aviation please make sure you complete the following forms and email them to Recruiter@AvsourceAviation.com, fax your forms to 859.282.8777 or mail them to the address above.

ALL FORMS AND PAPERWORK MUST BE COMPLETED AND RECEIVED BY HR PRIOR TO YOUR EMPLOYMENT START DATE.

    List of required forms/items:
  • Employees must review the Avsource Sign-Up Packet and sign all included forms. The Sign-Up Packet can be downloaded from the following link: Download Sign-Up Packet
  • A clear copy of your Social Security Card, current Drivers Licenses, and/or passport.
  • A copy of any/all licenses: e.g. A&P, FCC, etc
  • A copy of all training records/certificates
  • Employees must review the Avsource Aviation Alcohol and Substance Abuse Program handbook. Please sign and date the form located in the front page of this handbook once you’ve reviewed the information located within. The handbook may be downloaded from the following link: Download ASA Handbook
  • Employees must review the Avsource Employee Policies and Procedures Handbook. Please sign and date the form located on the front page of this handbook. The handbook may be downloaded from the following link: Download Employee Handbook
  • For employees that are eligible for Per Diem the Certificate of Permanent Residence form must be completed and the same address must appear on the I-9 and W-4 forms.
  • The employee portion of the W-4 and I-9 forms must be completed and signed. Section 1 is the only employee part of the I-9. If you need to update your W-4 or I-9 IRS form you can download these forms here: W-4 Form Download from IRS website & I-9 Form Download from the Department of Homeland Security website.

ADDITIONALLY: ALL EMPLOYEES MUST PASS THE PRE-EMPLOYMENT DRUG TEST AND BACKGROUND CHECK PRIOR TO STARTING WORK. EMPLOYEES ARE NOT ELIGIBLE TO START WORK UNTIL HR HAS RECEIVED ALL REQUIRED PAPERWORK.
